This is the manual that has the programming and master unlock codes for just about all of the remotes that look similar to the one pictured, which definitely have more buttons, but the codes should work regardless.
It is interestingly enough that I could not find, despite them having only short versions of them, the real manuals on the rogers/scientific atlanta websites. So I scanned each and every single page, including the front and back cover, for your viewing. This took quite a few hours to do, and all were scanned at 600 dpi.
